Our 5 steps to implementing Care
Initial Contact (Stage 1)
Upon receiving a referral or inquiry for services through the completion of a form on this page, you will be contacted by our Care Coordinator to schedule an appointment at a time that is convenient for you, your family or those involved in your care. A free home visit will be scheduled during which an assessment of your basic needs and environment safety assessment will be conducted and all of your questions answered.
Create Care plan (Stage 2)
Together, we will create a person centered care plan that addresses your individual needs, preferences and goals. This will serve as a guide to all who are assigned to your care. Our care plans are reviewed and updated as the situation changes to meet your current needs.
Caregiver Matching Process (stage 3)
Above and Beyond Nursing Services understands how important it is to match our clients with the right caregiver, which is why we put in time and consideration to ensure the caregiver assigned to you aligns with your personality and needs. All caregivers are screened, including background checks and professional reference verifications, to ensure your safety.
Exceptional Care Delivery (Stage 4)
We have a mission to deliver care that is truly exceptional, and we take advantage of every interaction with our clients to live out that mission. Our caregivers have received the proper training that enables them to deliver high quality and professional care according to your specific needs. Our caregivers will go the extra mile to ensure our client’s experience not only physical wellbeing but also an enhanced quality of life.
Continuous support and Follow up (Stage 5)
We provide ongoing support and regular check-ins to ensure your satisfaction and well-being are consistently being met. Once a caregiver has been assigned, our care coordinator or manager will check in periodically via phone calls or monthly visits to ensure you are satisfied with the services being rendered. The caregiver is required to document every care that is being provided to you and this document can be reviewed by client or family members for verification purposes.
